VAGINOSISWhat is Vaginosis ?This is a condition which affects the vagina (front passage). The vagina is normally full of bacteria which help to keep the secretions acid. This maintains a healthy environment. Sometimes there is a change in the numbers and types of bacteria. It is this which causes the condition of bacterial vaginosis. There may be no symptoms. If symptoms are present, they usually include a thin white offensive discharge. This offensive fishy odour is often worse after sexual intercourse. The discharge may cause an irritation of the skin. Bacterial vaginosis does not cause the redness or swelling which is found in candidiasis (thrush) and trichomoniasis. How does Vaginosis occur ?The reasons for the change in vaginal bacteria are not understood. The bacteria associated with this condition include Gardnerella vaginalis . Bacterial vaginosis is not thought to be sexually transmitted. Why does Vaginosis occur ?No-one knows. It can come and go for no reason in women who are having sex and also in women who are not. It is more common in black women, in women with an IUCD, and in those who smoke. Treatment InvolvedThe doctor will probably make the diagnosis on the basis of your symptoms. If the condition keeps returning you may be referred to a department of genito-urinary medicine (special clinic, STD clinic). INVESTIGATIONS A specimen of discharge is examined under the microscope for a certain type of cell. The doctor will check the acidity of your vagina. He may send a specimen of discharge to the laboratory to culture the bacteria. TREATMENT Treatment usually consists of a short course or single dose of a drug called metronidazole. It is not necessary to see or treat the sexual partner unless the condition keeps recurring. During Treatment of VaginosisSymptoms usually settle within a week. 20% of women may need more than one course of treatment. It is common to get some nausea when taking the tablets, which taste unpleasant. It is best to take them after meals. Alcohol must be avoided whilst taking the tablets as it reacts with metronidazole to cause vomiting. Although there is no evidence of harm, it is better to avoid metronidazole in the first 3 months of pregnancy. After TreatmentBacterial vaginosis frequently recurs and treatment may need repeating. Further investigations may be necessary, such as a screen for sexually transmitted infections, if the condition keeps recurring. If Vaginosis is left UntreatedBacterial vaginosis often resolves on its own. If not, then the symptoms simply persist. Effects on family of VaginosisIf the condition keeps recurring, it is advisable for the sexual partner to have a check-up. This is best done at a department of genito-urinary medicine (special clinic, STD clinic). |
